Reimbursement Form: to be used if you have an invoice to be paid; money that needs to be reimbursed to you for a committee expense
Cash Needs for Events Form: to be filled out before an event if you need cash boxes, checks for vendors, tips, etc.
PTA Cash And Check Deposits Form: Anytime you are turning in checks or cash, you will need to turn this in with the funds - it is best when used on the computer since the spreadsheet does the math for you. I will not be able to accept funds without the committee chair (or designated committee member) counting the money and signing the form. This is an additional internal control that has been set forth by the Audit Committee.
501 (c) 3 Determination Letter: this letter proves the PTA's tax exempt status. Use this letter only if it is requested by a corporation or sponsor who may be making a tax-deductable donation and needs this form to get their tax deduction.
Tax Exempt Form: this form is used if you are making larger purchases for a PTA event in order for the vendor to waive the tax - the check should always come from the Westbriar PTA and not from your personal account.
PTA Letterhead: please use this letterhead template for all correspondence in association with PTA activities.
Event Planning Sheet: Complete this form to request use of the school for a PTA sponsored event. Form should be turned into the Assistant Principal at least 2 weeks prior to the event.
Community Use Form: Use this form if you need to reserve space at Westbriar for a NEW program or enrichment activity. Turn in to the Assistant Principal at least two weeks prior to advertising your event and/or contracting your event.
Parent Sign Out Form: This form (or similar) must be used for parents to sign out their children from an afterschool activtity.
